We have an open list and welcome requests for registration from patients living in or moving to the practice area.
You can register by:
- completing a combined online registration and health form or
- completing a form available from reception or
- downloading and printing a registration form here
You will need to complete any paper forms by hand and bring to the surgery along with proof of address e.g. bank statement/utility bill.
New patients are asked to complete a brief questionnaire when applying to join our list detailing past medical history and current medication.
Please inform us immediately if you move or change telephone number.
In order to provide the level of service that we require, we have a practice boundary. In some cases when you move house this will take you outside our catchment area and we will have to ask you to re-register with another surgery.
You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
We recommend that new patients undertake a health check with a practice nurse.
